Contract for heart murmur puppy

I wrote this out for when I sell the puppies and I want to see what you think (there are some blank spaces at the moment but will be fill in later this week). This contract is seperate to the neutering/spay contract.

I have been given a copy of the written diagnoses from the vets and other information that explains what a Canine Heart Murmur is.
I will take full responsibility of any medical expenses that may or may not occur in the future due to this condition.
I understand that I purchased this puppy at a reduced price due to the Heart Murmur.

Looks good, have to congratulate you on not only having puppy tested but adding an adendum to your contract regarding the issue.

Myself, I would put a clause that puppy must be retested and results submitted to your vet at the age of between 12 & 18 months and that if the puppy dies between that time you will replace with another puppy and or refund at least a portion if not the full amount. I understand that they're taking the pup knowing their is an issue, but to love a bub and, even knowing, and have it pass young IMHO it's right as a breeder to replace at that young an age. Five or Six years down the road, not so much.
