Unhappy Update #1 -- Louise is worse.

Well.. for those of you curious and anxiously waiting. Louise has taken a decline.

Yesterday she seemed relatively better, no further vomiting or diarrhea. Until this morning.. more liquid diarrhea. I called into work to get her bloodwork results... not good. She now has elevated kidney values. Her BUN, Creatinine, and Phosph. are all up. I brought her into work with me today to start fluid therapy and see how she did.

As the day progressed, I noticed her breath was toxic smelling. Like metal, which usually means uremia. Which can either work together with kidney failure or blood in the intestines. Well, she seems to have a little bit of both going. In the afternoon when I walked her outside, she had projectile diarrhea that was entirely blood. It's been snowing here, so it was very clear it was entirely red.

I rushed her over to my previous hospital, as they are speciality/ER and I admitted her for fluid therapy and to have an internal medicine specialist on her case. We ideally wanted to perform ultrasound this afternoon/early evening, with no luck.

We repeated some bloodwork on admit, and her values have slightly decreased -- although still elevated.

We have submitted more tests -- lepto titers, urine culture, and tomorrow will be ACTH Stim and Aldosterone.

For the ACTH Stim/Aldosterone we are trying to rule out if this is her Addison's. It's very difficult to rule out, as she is an atypical addisonian. Meaning her electrolytes have never been imbalanced. Currently the ratio is still normal, however her sodium is very decreased. (Which works with renal failure, but could also work with dehydration/addison's)

Tomorrow is also the ultrasound. See what kidneys look like? Maybe GI tract? Who knows.. hopefully there might be a little answer in there?

She will be at least admitted for 24 hour care, possibly more depending on how she does overnight. I suspect more?

I am a little lost with how far to go.. her body is clearly failing her. This and her previous conditions. I am trying to stay positive, and the internist seems confident in being able to treat. But knowing what I know in the field, I am a not entirely optimistic. Her prognosis is definitely guarded at this point. Even if she does fall with the diagnosis of renal failure, I am not sure how far to push the dog. I will see how she makes it through this, what the outcome is?

I will update again tomorrow evening, pending any answers.

I'm sorry to hear that Louise isn't improving but we know she's in the right place getting the best care. Sometimes it's hard knowing what you know because of the field you work in and that can play havoc with your mind. Hugs to you and Louise & Harvey sends her lots of licks from his crate...

I am sorry. I will be thinking of you and Louise hoping they can pinpoint her illness and her condition improves. You have been through so much with her, I am glad she has you for a mom.
